I would tell him. You have had 5 dates. Obviously he does not love you yet at this point and superficial things like this could be a deal breaker to some people. Wouldn't you rather have him disappear on you before than after sex. And you can tell a lot by how he reacts. If he says that it is no big deal and comments that he doesn't have a perfect body either you know that he is a keeper!
