Had to Abruptly Stop Taking Cymbalta After Surgery; Having Medication Withdrawals

Hello,

I just had the Gastric Sleeve on 11/15/16. I had to stop taking Cymbalta cold turkey after the surgery because it is a delayed release capsule and you can't take those after surgery. The pill doesn't come in any other form. I am having some withdrawal symptoms such as anxiety and bran zaps. Did anyone else have to do this? Any advice or suggestions on the process?

You shouldn't just suddenly stop taking this medication. Call your Dr and get something for anxiety also am pretty sure you can open the capsules and mix then with liquids so ask about that too. As far I'm concerned it would be appropriate for you to go the urgent or emergency dept to be seen for this.

Where did you have your surgery? This should have been addressed pre op.

Ah! That extended release thing is a MYTH. You poor thing. Call your PCP or doctor who put you on it asap to see how you can restart it. You should NEVER stop anti depressants cold turkey!

I had to get off of cymbalta and had the same symptoms you're describing, even the lowest dose was not low enough. I ended up opening the capsules and counting out the little balls to walk the dosage down. One option I didn't know about then, but according to several docs works great is to get switched to something like Prozac which eases the ramp down. But yeah, double check on the extended release, I've not seen any literature about it being contraindicated. You can't crush them is all. My doc let me take small pills and capsules from day 1. Your doc should be able to give you some options.

I'm started walking the dosage down today with counting the beads. I have other anxiety Meds that are helping and I have to get off other Meds before and the withdrawal was much worse than this. It's not capsules but it's the extended release capsules that he doesn't want sitting on my stomach. It was Dr. Bonnor in Katy. Other than the medications, I would say everything else went good. I'm trying the bead thing so we will see how it goes! Thanks for the advice! I really appreciate everything!!

Agreeing with everyone else - be VERY careful with anti depressants, I'm speaking from experience. Hormones and frustration will make you nuts and depressed after surgery. Please keep your baseline good. Maybe your PCP can help??

Yea. I've been through it before with another medication. I started back on the med and will wean myself off slower. I have Ativan so that's helping with the anxiety. I have a follow up with him next week and will be talking to my PCP if it doesn't get better. Thank you!

I was on long acting oxycodone during my pre op phase and was told by the WLS NP that I needed to be completely off it before the surgery. I reduced my dosage from 30 mg 2X day to 10mg 2X day and supplementing with short acting. I was planning on going to short acting dilaudid right before surgery.

When I saw the surgeon for the pre op she told me there was no medical reason to stop taking the extended release. Apparently the NP I saw has issues with narcotic pain meds...

A topic for another day: why are medical providers with personal agendas allowed to present their OPINIONS as FACTS?

Interestingly, the change in diet reduced my cronic inflammation to the point where I was almost completely off pain medication too weeks post op!

Back on pain meds now because of inflamed triennial nerves (very painful!) but hoping it's going to be very short term. Got the morphine pump all worked out today.

I just reread this, and good thing as the "auto correct" changed "narcotic PAIN meds" to "narcotic PARTY meds and " cronic inflammation" to "comic inflation"!

Yea. It sucks. It really does but I'm ready to be off of it. It's increasing my restless legs problem. The zaps are awful but it's a good thing I haven't been on it that long. Thank God. I'm glad you got through it and thank you for the encouragement.

I should add I was under the care of Doctor during this process and temporarily took Wellbutrin to take the edge off. Call your doc and see what they recommend.

I feel your pain, I went through the exact same thing. I had to switch medications because of the time delay stuff. I take Xanax as needed now.

I was also on Cymbalta. My doctor changed my prescription to Effexor. She said it was in the same family but is a tad bit bigger than an M&M, which is my doctor's guide to use if you swallow your medicine. I have to cut it up in quarters though because I can taste halves going down! I tried to ween myself off after surgery because I just didn't want to have to swallow it. Ran into trouble right away!

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
