Can Gastric Bypass Surgery Put Diabetes in Remission?

A study by David Cummings et. al. published in Diabetalogia investigated the mounting evidence indicates that Roux-en-Y gastric bypass (RYGB) ameliorates type 2 diabetes.

32 adults were divided into 2 groups. In the ILMI [intensive lifestyle and medical intervention] group, the individuals performed ≥45 min of aerobic exercise 5 days per week, a dietitian-directed weight- and glucose-lowering diet, and optimal diabetes medical treatment for 1 year. In the RYGB group, the individuals received Roux-en-Y gastric bypass (RYGB).

Diabetes remission at 1 year was 60.0% with RYGB vs 5.9% with ILMI (p = 0.002). Compared with the most rigorous ILMI yet tested against surgery in a randomised trial, RYGB yielded greater type 2 diabetes remission in mild-to-moderately obese patients recruited from a well-informed, population-based sample.

I am almost 3 years post-op RNY surgery. I had Type 2 diabetes and took 2 types of medication prior to surgery. When I left the hospital 2 days after surgery, my diabetes went into remission and I went off my meds. It has stayed in remission ever since. Many individuals on this website report similar results. But a few don't. From prior posts, one of the variables for success appears to be the length of time the individual had diabetes prior to surgery. So one of the interesting points of this study is:

The groups were equivalent regarding all baseline characteristics, except that the RYGB cohort had a longer diabetes duration (11.4 ± 4.8 vs 6.8 ± 5.2 years, p = 0.009).

My A1C at pre-op was a 6.5 and I have not taken any meds since 2 months prior to my surgery and have not had a high BS testing since then...I would have to say it assisted me into going into remission..but I think that is only because of the different food choices I have made. Pre-op was on Atkins (carbs less than 20) and since surgery only meat and small portions of green veggies so I think that has more to do with it.

Prior to surgery I was type 2 diabetic taking 2 medications including insulin and blood pressure medicine. Since surgery, my diabetes has been in remission. No medicine whatsoever and just had blood work done a couple weeks ago and my A1C is in the normal range

My A1C prior to my pre op diet was 9.5 and I was on an insulin pump, metformin and Januvia. 4 weeks into my pre op diet I dropped 40 lbs and my blood sugars ranged in the 80's. And A1C dropped to 6.9. My endocrinologist took me off my insulin pump and all diabetes medications even before surgury. Today I'm 6 days post op and my numbers are below 100. I follow up with my endocrinologist ar the end if the month so I'm hoping my diabetes will stay in remission.

I had normal blood glucose levels, without medication, within 48 hours of my RNY. I'm now 9 months out, and continue to be fine.

My sugars stabilized post op but I can't say I am cured. No meds, but doing alot to keep bg levels below 100. Gone up after I returned to work, had been in 70s 80s right after surgery.

I am a type 1 diabetic so while my upcoming surgery is unlikely to cure my diabetes, it could make significant differences to control. Some studies show that type 1 diabetics who are overweight have a much harder time keeping their sugars well controlled and this seems to improve after RNY surgery thus improving their overall health because of it.

@@tonetone me too, great till about 3 weeks after surgery now they are around 130, still in diabetic range, I'm hoping they will go down to normal range again the more weight i lose. Since I stated my pre RNY surgery diet in April '16, I have lost almost 50 lbs. Not taking meds now

Well I am 3 weeks post op now and still on insulin ... I am taking half the daily dose I used to but still need it or my sugars go up even if I eat and drink nothing. I do find that I don't need the fast acting insulin with meals because I don't eat enough to affect my sugars significantly.

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
