My surgeon office charges $35 to fill out FMLA and STD forms..please advice..did your job fill out form or doctor office?

My doctor completed my FMLA paper.

Mine charged for it.

My OB/GYN office charged me(for c-section), my Reproductive doctor did not(tube surgery), I don't know about the surgical office yet(gastic sleeve). Still waiting on my date.

I sure hope not as much as they are getting paid for the surgery , I'm still awaiting my date I was just approved by my insurance .

I'm going to call Cigna, that's the company my job use for FLMA/STD to process forms.

Mine didn't, but my kids pediatrician charges for school physicals paperwork and drivers license medical clearance. They're allowed to do it, I think it's bad PR personally.

At my surgeons office the $600 program fee covered the completion of ur FMLA OR STD forms, group nutrition class, life time of group session, and on going support from dietician.
