Does anyone have a template I could use regarding a letter of medical necessity that my doctor has to send to my insurance? What has everyone's experiences been in dealing with your insurance and these types of letters?

Won't the dr know how to write a letter of medical necessity??? I'm going to my first surgeob consult Monday and tuesday I have made an appointment with my PCM to do whatever else needs done, the lab orders letter of medical necessity, I just figured the dr would know what to do or is this just so you all can see what it will look like? If thats why then I would like to see one too:)

Ask your bariatric surgeon. Mine came with the lap band "packet" of materials that I would need before the surgery. You know, the hoops you have to jump through. What they required was your yearly weight for the past five years.
