Does anyone have a template I could use regarding a letter of medical necessity that my doctor has to send to my insurance? What has everyone's experiences been in dealing with your insurance and these types of letters?

wow.. I am going thru TRUE RESULTS and I didn't need any of that... They sent it all, they did all the leg work and I did most of my testing with them....I saw them in Dec and was ready to go at the end of dec and the only thing that is holding me up is that my med turn over for the new year is March... so I will be banded in march....I SOOO love how everyone helps in here... I love the chat line too... soo much help
