6 Weeks Out and Struggling

This past Monday was my 6 week anniversary, and overall, I feel really good about my progress. I'm down 40 lbs since my surgery date, and a total 85 since I started dieting on New Years Day. The problem is the last 2 weeks, my stomach has not been very receptive to just about anything. Just about every other meal has either quickly come back up or stayed down but left me with heavy dry heaves and the infamous slimies (although I prefer the other term of foamies... sounds slightly less disgusting.)

Recently it got exceedingly bad, as I spent the entire weekend without the ability to take anything in. I became so nauseated that even Water would make me feel ready to run to the nearest toilet or garbage can. By Monday, I had become so dehydrated that I hit the terrible catch-22 where I was more nauseous from the dehydration, but if I tried to drink, I felt more nauseous still. Called my surgeon and he instructed me to go to the ER where I spent 5 hours on IV fluids, 2 liters of straight saline and a banana bag (saline with thiamine, folic acid, magnesium sulfate, and a Multivitamin mix.) I felt really good afterwards, but today, I've been feeling a bit nauseous again and threw up a little this morning even though I hadn't eaten anything yet.

Hopefully this all passes soon, it's driving me crazy.

Some people experience problems with ulcers after surgery. According tot he internet:

Nausea and vomiting are the most common complaints after bariatric surgery, and they are typically associated with inappropriate diet and noncompliance with a gastroplasty diet (ie, eat undisturbed, chew meticulously, never drink with meals, and wait 2 hours before drinking after solid food is consumed). If these symptoms are associated with epigastric pain, significant dehydration, or not explained by dietary indiscretions, an alternative diagnosis must be explored. One of the most common complications causing nausea and vomiting in gastric bypass patients is anastomotic ulcers, with and without stomal stenosis. Ulceration or stenosis at the gastrojejunostomy of the gastric bypass has a reported incidence of 3% to 20%. Although no unifying explanation for the etiology of anastomotic ulcers exists, most experts agree that the pathogenesis is likely multifactorial. These ulcers are thought to be due to a combination of preserved acid secretion in the pouch, tension from the Roux limb, ischemia from the operation, nonsteroidal anti-inflammatory drug (NSAID) use, and perhaps Helicobacter pylori infection. Evidence suggests that little acid is secreted in the gastric bypass pouch; however, staple line dehiscence may lead to excessive acid bathing of the anastomosis. Treatment for both marginal ulcers and stomal ulcers should include avoidance of NSAIDs, antisecretory therapy with proton-pump inhibitors, and/or sucralfate. In addition, H pylori infection should be identified and treated, if present.

I've definitely followed every instruction, as listed above, very meticulously. If anything, I've been even more cautious then that, I very carefully measure all of my food out and cut everything up very small before I begin eating. I pretty much time myself as well in order to best follow the 30/30/30 rule of drinking and meal time. Many of my meals do end up being a little less then the instructed 3 oz, because I haven't had much of an appetite and I don't enjoy most anything that I have eaten, even things that I used to love pre-op.

I have been doing some research and if things don't improve with the dehydration after this week, I will reach out to a few of my doctors in order to determine if some of my medications may be the problem. Aside from the bariatric prescribed multivitamin, Calcium, protonix and gallbladder medication, I also take anxiety medication, cholesterol meds, and I recently also started hormone replacement therapy. One of the hormone medications, spironolactone, aside from it's use in helping with transgender patients, is that it's primary a Water pill. Due to this, it is possible that the medication is partly at fault for my dehydration, although it would not account for the repetitive nausea, as I was already getting that just before I started the new medication.

I guess time will tell and we'll see what happens as I progress a little and hopefully I can start keeping things down more often.

Hey friend! Sorry you hare having such problems. Did the doctor prescribe any dissolvable anti-nausea medication? I used it a lot in the first 2 months. One thing I did to help with dehydration was I drank Pedialyte for kids. It's not the best tasting but my doctor recommended it to me when I wasn't getting enough fluids in.

No new rxs as of yet. If it continues then I will ask about alternatives. I actually have since nausea meds at home from an unrelated issue a few months earlier. I'll have to see if it's still good and maybe I'll give it a shot.

The nausea meds saved me a lot of misery! I hope they work for you.

You are probably dehydrated which causes nausea. Definitely refill your script the nausea med it helps you have to eat dear and hydrate. Or you will be-back at the Er and call your surgeon and let them no also

I would suggest a plant-based diet. Try drinking only Water and plant based Protein Shakes or plant based Protein Powder with almond milk instead of dairy. Try quinoa and lentils & or nuts for additional Protein. Eat fresh fruits and vegetables or frozen vegetables. If this works for you then slowly start adding in small amounts of meat preferably lean protein. Don’t eat any fried foods or added oil in foods. Grilled lean poultry & fish/Shrimp/seafood are great & easy on the stomach.

85 pounds? That is NUTS

I had my surgery Dec 11 and I have only lost like 72 pounds.. Men lose much faster than women, so you have a little leverage but still. Good is good and you have done great

I am closing in on 6 weeks as well. I too was having issues eating/drinking. You know what worked for me? The Mucinex D that my Dr told me to take if I ever got nauseated or the foamies.

It is kind of gross but when our stomach is larger, there is plenty of room for the sinus drainage that gets dumped there everyday. Not the case when most of our stomach is removed. When the tummy is overfilled with sinus drainage, it comes up as foamies. Give it a shot. Worth a try for sure and good luck!

      Two years out. Lost 120 , regained 5 lbs. Recently has a bout of Norovirus, lost 7 pounds in two days. Now my stomach feels like it did right after my surgery. Sore, sensitive to even water.  Anyone out there have a similar experience?

      Am I the only struggling to get weight down. I started with weight of 297 and now im 280 but seem to not lose more weight. My nutrtionist told me not to worry about the pounds because I might still be losing inches. However, I do not really see much of a difference is this happen to any of you, if so any tips?

      Well recovering from gallbladder removal was a lot like recovering from the modified duodenal switch surgery, twice in 4 months yay 🥳😭. I'm having to battle cravings for everything i shouldn't have, on top of trying to figure out what happens after i eat something. Sigh, let me fast forward a couple of months when everyday isn't a constant battle and i can function like a normal person again! 😞
