Switching from the Sleeve to the Roux en Y

Ok two days before my surgery I changed from the Sleeve to the Roux en Y. Who else has done this or why did you choose the Roux en Y? My reasons are I believe it will be better accountability for me and extend my success past a year. What are some of y'alls reasons for choosing Roux en Y and maybe switching like I did?

I switched as soon as I got approval from insurance. I had GERD issues and didn't want to end up having to get a revision from the sleeve to RNY in a few years. It's very common for those with previous GERD who are sleeved to have to get it revised to RNY within a few years. My surgeon had recommended it at first but I was hesitant. Plus, the amount of weight lost is greater with RNY.

I have GERD too....and Barrett's Esophagus. Biopsies came back negative. I saw my doc on wed to go over my 24 hr acid test and barium swallow. She says with everything going on, I need to revise my sleeve to a roux n y. she will also be repairing my hiatal hernia and doing surgery near my esophagus. I only had from wed till next Monday's surgery to grasp the concept and be emotionally prepared. She was booked solid till end of summer for surgery but had an opening for surgery next Monday. she didn't want me to wait and suffer any longer, which is why I am having surgery on Monday. I love my sleeve because I know how much I can eat. I'm nervous about my stomach getting much smaller.

I just had a consult w my surgeon today .. he says either is fine for me .. cos no gerd issues or diabetes .. but diabetes runs in big in my family history.. also my BMI is 40 which I was surprised he suggested bypass as an option as well ..i heard it can be more risky ..& sleeve less invasive ... which I know surgery is surgery they are all invasive .. I'm only rethinking my decision to bypass versus sleeve because the long-term success.. Can anyone share there thoughts on this ?

I asked my doctor in his practice where he saw the most long time success and he said the Roux and Y. He told me the accountability it puts in place with dumping if you are not eating right is a hard reminder it is in place. I needed that accountability. The Roux and Y is also reversible if you ever want to change. My docs patients also lost more weight on the Roux and Y too. I"m 3 days post surgery and operating fine. Down 11 lbs since surgery, walking a mile a day and getting out of the house. My only regret is not doing it earlier.

Thanks for sharing.. wow 3 days 11lbs is fast .. congrats!
I do like that this surgery is golden standard and is proven successful with all the data in place.
I read that you lose for two years with RNY versus 1 year w sleeve ..
what was your starting BMI ?

I'm also I just thinking about comparing complications & Long Jevity with two different surgeries that's my biggest thought. .. which is better ?




BTW
My sister had RNY back in 04 before sleeve was popular she did fine w it & lost over 100lbs & kept it off .
I like the accountability it does have w dumping but it's so scary & painful..

41 or 42 is my BMI. I think both surgeries have their benefits and you can be successful with them. I think a lot of it is where you are at. I want to lose 140 lbs. I want to be lean and healthy. Dumping is something I'm not looking forward to but I want to try to cultivate the discipline where I don't go there on purpose. I think checking it out and finding your surgeon that you trust will guide you the right way.

I think this decision is just a matter of yes where your at & how much we want to lose .. I just suggested sleeve to my Dr cos of not changing intestines but By pass has more weight loss w good data.
He says I can be approved for either one it's a matter of choice .
So far we decided on sleeve but yes I'm thinking of changing to By pass .. just looking to be very heathy too & get a low BMI as well. I was active w exercise & Lower BMI over 10 years ...& I am still somewhat active.
gained over 100lbs having triplets & have had a hard time losing it all the weight .
It seems I lose some but keep regaining it back .
I need a way to stay accountable.

Good luck on your journey sounds like you have a good plan in place .

I had a sleeve 5 1/2 yrs. ago. However I had a lot of repair that had to be done then at the same time to my diaphragm. Before surgery 2/3 pf my stomach was betweeno my lungs headed for my heart so the doctor had to perform a sleeve to get my stomach back down without destroying my diaphragm. I had so many stitches in my diaphragm he could not apply the mesh to keep another hiatal hernia from starting. About 6 months ago i started getting bad reflux again. I had an upper gi endoscopy and discovered a small spot of Barretts that came back negative, but i was refluxing throughout the procedure bad. My hiatal hernia is starting to come back. My doctor said I can have just that fixed or have a gastric bypass and solve the acid problem for good and of course no more Barretts would come. So i am having the Roux en y gastrojujenstoma bypass on June 12. Can't wait. I had no problems with that surgery and he told me this one would basically be a piece of cake after the first. Have not gained any weight back in 5 1/2 years. If you have a hiatal hernia repaired with mesh the same time as your sleeve, I think your acid should be OK.

I too originally chose the sleeve. Had the surgery scheduled and everything. In fact, I would have already had my surgery over & done with if I'd proceeded with the sleeve, and I think (ok, I know) my family would have felt more comfortable with that decision. They feel it's safer, easier recovery, and lower complications.

But my bmi is just over 50, and I'm extremely concerned I will not lose all the weight with the sleeve. I'll be honest. I'm not going through this to lose "most" of the weight. I'm bound and determined I will get to a healthy weight for my bmi come hell or high Water. I have some issues now with acid reflux, and although I've not been diagnosed with GERD, I'm afraid the sleeve would exacerbate it. And I don't want to have to have one surgery, only to have to have the RNY further down the line. I think the dumping (if I'm one of the ones who have that) will be a bonus for me given my particular issues with food. I guess my biggest reason is....my head and my heart are in agreement that i truly feel this is the best choice for ME.

Please be aware I don't think one surgery is better than the other, I don't necessarily agree with my family's opinion, (insert all the other caveats and disclaimers here). All I can do, and all any of us can do, is make the best choice we can for ourselves, and be supportive of each other whatever that choice is.

Since I started this journey I always wanted RNY. For some reason I started second guessing myself & wanting the sleeve. As soon as I got my upper endoscopy & found out I had mild reflux I stuck with RNY because I don't want to mess with GERD & have to revise later.

I'm actually considering going the other way. The surgeon suggested sleeve but I told him I must have RNY, so he put me down for that surgery. Now Im having second thoughts and I have scheduled another consultation and I am planning on telling him I want the sleeve....I dont know which one to do!

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL

      Still purging all of the larger clothing. This morning, a shirt that I ADORED wearing ended up on top. Hard to let it go, but it was also hard to let go of those habits that also no longer serve my highest good. Onward and upward!
