United healthcare weight loss form

I was told I needed weights recorded from anytime in the last 5 years from any doctor. I filled the form out myself and faxed it back. Now they're saying the form needs to be signed and more recent. Any tips on how to get this form filled out ? I don't regularly see my PCP but have seen my GYN a lot in the past 3 years.

My approval is waiting on this only.

It sounds like you need to have your GYN and/or your PCP fill out the form.

So I sorted this out. Had to print multiple forms and deliver them to my different doctors. I have a requirement of 6 weights to be logged. 3 came in so 3 more to go. I explained to my PCP that I wish to have this done. She was supportive so I'll continue to see her. Looking then to have my last requirements met by end of July. Yay!

Interesting follow-up. I was allowed to use the nutritionalist consultation as a weighin but during this time of gathering the weights, one of my earlier one expired - outside Insurances 2 year ( previously told it was 5) time span. Crazy. I'm trying to see if they will take that earlier one as it qualified when we submitted the paperwork.

      Am I the only struggling to get weight down. I started with weight of 297 and now im 280 but seem to not lose more weight. My nutrtionist told me not to worry about the pounds because I might still be losing inches. However, I do not really see much of a difference is this happen to any of you, if so any tips?

      Well recovering from gallbladder removal was a lot like recovering from the modified duodenal switch surgery, twice in 4 months yay 🥳😭. I'm having to battle cravings for everything i shouldn't have, on top of trying to figure out what happens after i eat something. Sigh, let me fast forward a couple of months when everyday isn't a constant battle and i can function like a normal person again! 😞
