Gallbladder removal and bile acid malabsorption (anyone?)

Sorry, this is a bit long and it's getting late (forgive my typos). To avoid the tl:dr reaction -- does anyone take cholestyramine and how did you take it after your WLS surgery? See background details below for more info. Thanks in advance for your comments.

Background --

My gallbladder was removed many years ago as I was finishing college, approximately 18 years ago. Since that time I've been living with chronic bathroom issues, namely diarrhea, that would present within minutes of eating lunch (generally the first meal of the day). It didn't matter what I ate - bread, Soup, iced tea, whatever. I might go once after eating or I might go for the rest of day (too many times to count).

The diarrhea issues would last for several months and then might go away for several months or a year. However, the problem always returned.

I went to numerous doctors about this problem and was always told something like, "you have IBS, or you have spastic colon" or something similar. Last year, I went to my (former) primary care physician armed with my internet diagnosis of Bile Acid Malabsorption. My physician didn't listen again and just said I needed more Fiber. Long story short, someone recommended their gastro doc to me and I went. The gastro doc said, "I know exactly what the problem is..." naturally I had to ask is it "bile acid malabsorption -- the result of my gall bladder surgery?" Ding Ding. That was the answer.

I drink this gritty powder (that does not dissolve) called Cholestyramine (once a day) and my life has changed dramatically - problem solved.

Has anyone else with a sleeve or other WL surgery had this gallbladder related issue and were you able to continue taking your cholestryramine after surgery?

Following. I suffer for the same thing! My surgery is 2/19 for WL. My mom, who had gastric bypass, and I call it "gallbladder dumping syndrome". So uncomfortable! The only time I didn't experience it was while pregnant - it made my stools normal!

I am very interested in knowing more about this issue! I have had my Gall bladder removed 11 years ago and My bowls play up all the time !! Even a small take out coffee has me running to the toliet ! I am worried after having my Sleeve done that this will get worse !

I had the same issue after my gall bladder surgery 7 years ago. I found a pill form of cholestyramine called Welchol and was able to convince my doctor to let me try that instead of the powder. It has worked well all this time and also made my cholesterol amazing. (It's a cholesterol medication.) Before my bypass, I asked my surgeon if it surgery would effect this either negatively or positively. He said probably neither. He also said only about 3% of people get bile salt diarrhea after gall bladder surgery. Anyway, mine has actually been better. I only take 1/2 a pill now and only once every couple of weeks instead of a whole one every day.

I had my gall bladder removed 5 years ago and suffered for about 4 years with bile acid diarrhea before starting on the cholestyramine, and it works great! Wish I had tried it much earlier. I also asked my surgeon if I'd be able to continue taking it after sleeve surgery and he said he didn't see any reason why not. He did mention that most patients complain of Constipation after surgery, so this could end up being a non-issue. In fact, this has turned out to be the case. I had my sleeve done on 1/07/15 and so far I've only taken 1 dose of the cholestyramine, simply because I thought I "should". I haven't taken any more in the past 15 days and have no diarrhea problems. Now, will that change when I'm able to eat more? We'll have to see. I'm just grateful that the medicine is available for us "3% ers"!

i am going to give the Medicines a go will chat with my Doctor next visit ! 11 years of running to the toliet, I would love that part to be in the past ;)

Yes!!!! It took me Eight years post gallbladder surgery to find an answer to my post gallbladder bile diarrhea. Mostly because it hasn’t been consistent. I could go months without having any and then one day a sip of Water can send me straight to the bathroom. I was sleeved two weeks ago, and haven’t taken my cholestyramine for over a year pre op because I wasn’t having issues. I find it’s much worse for me when I’m stressed or overly tired. Also, the powder is gross, but luckily when I need it I can get by with 1/2 to 1/3 a packet. I was actually planning on asking my surgeon this week about restarting it because I’m having symptoms again post op. I was almost hoping to be constipated post op or a nice change of pace! Lol!! (Sorry, dealing with this actually put in a depression for awhile because I felt chained to my house as I never knew when it would hit so I try to find humor on it when I can)

side note- it may have actually ended up saving my life because the gastro that diagnosed me insisted on a colonoscopy (at 33) and found a significantly large pre-cancerous polyp. I ended up having four colonoscopies in 18 months and am coming up for my two year check this Spring. Point being, if I had waited until the normal age to have one, it probably wouldn’t have been precancerous anymore.

      April Surgery
      Am I the only struggling to get weight down. I started with weight of 297 and now im 280 but seem to not lose more weight. My nutrtionist told me not to worry about the pounds because I might still be losing inches. However, I do not really see much of a difference is this happen to any of you, if so any tips?

      Well recovering from gallbladder removal was a lot like recovering from the modified duodenal switch surgery, twice in 4 months yay 🥳😭. I'm having to battle cravings for everything i shouldn't have, on top of trying to figure out what happens after i eat something. Sigh, let me fast forward a couple of months when everyday isn't a constant battle and i can function like a normal person again! 😞

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.
