I was wondering how detailed the diet history sheets have to be, I have well over 5 years of a BMI over 35 with 3 comorbidities. However with the diet history I can't remember the exact years and weight loss and gains from all of the Non-Physician supervised diets. I do have medical records from Physician Supervised weight loss however. Please can someone give me any advice on filling this out. I would really appreciate the help.

I would like to see replies to this too. I have found a pcp that wants to help me get the surgery done however i dont have records of every diet ive tried because i couldnt afford to go to the doctor every month to find out i was failing.

I just wrote all of the diets I've tried and what I lost and what I gained back during what year. I basically just tried to remember best I could...

As for medical I just had my pcp forward all my yearly physicals that had my weight documented.

Thanks That really helps, I have been on diets for the last 8 years so to remember them exactly would be exhausting. Thanks for the information!

This is like torture remembering diets, weight loss, weight gain, especially when I have been in a perpetual state of yo yo dieting for the last 8 years. I have my appointment with the surgeon on January 7th, I will let you know what he says about my diet history sheets. I will provide you with this information, hopefully it will help so you will have an easier time. :o

My doctor told me they don't really look at them beyond making sure you have actually tried to do something to lose the weight.

Your 5 year weight history from your doctor is the one that counts. Now if you are fudging numbers you don't want to say you were 300 at the same time your doctor says you were 250... But honestly weight fluctuates and you can be off a bit.

Thanks for the information, I have been seeing my current doctor since I moved to Texas 8 years ago (that's when the weight began to skyrocket out of control) :wacko: So my weight is well documented, the problem was the diet history and Non Physician supervised weight loss sheet, wanted every diet I have been on Yikes!! I have been sitting here staring at the paper wondering where do I begin, and hoping the information I do remember is sufficient enough. My anxiety level has gotten better now. Thanks for the info every bit helps, I have been soooo stressed over this.

For the 6 months program I'm in I just filled out 1 long page of diet history. With the weight I loss. Lol it was eye opening of how many diets I've tried. But then I for just in case started retrieving written data from a physician supervised diet that I been on and WW.records etc. So far haven't been ask for them yet! My BMI is 42.8,

I am hoping that I have it that easy, still working on mine. They want the city I was in when I started the diet how long I stayed on the diet and beginning and ending weight. Whew this is taking so much time and effort I will force them to look at them if they don't :) Thanks

My program may end up taking 9 months. It is a 6 month program. Lol. But as the 7th month is rolling in now I'm appreciating the time I'm putting in. It is preparing me better. Cause i had years and years bad eating, new diets, new gym memberships etc. Yea, I was anxious too. Now I'm getting exited. But this my opinion. Hang in there. You are worth it.

I don't have a time requirement, I just have to have one Nut and one Psych visit. The office coordinator said when I came in on the 7th, that's my first Md visit they then submit the paperwork to the Insurance. She also said that approval with Anthem BCBS is very fast, she said after they receive the go ahead I can schedule the date. I have been exhausted and nervously excited just waiting to get this over with! It's nice to see you are at the end of your preop phase. You have Real patience in regards to the 6 months. I would be pulling my teeth out by now. Well good luck can't wait to hear your post op stories. :D

      April Surgery
      Am I the only struggling to get weight down. I started with weight of 297 and now im 280 but seem to not lose more weight. My nutrtionist told me not to worry about the pounds because I might still be losing inches. However, I do not really see much of a difference is this happen to any of you, if so any tips?

      Well recovering from gallbladder removal was a lot like recovering from the modified duodenal switch surgery, twice in 4 months yay 🥳😭. I'm having to battle cravings for everything i shouldn't have, on top of trying to figure out what happens after i eat something. Sigh, let me fast forward a couple of months when everyday isn't a constant battle and i can function like a normal person again! 😞

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.
