Pcp Letter Of Medical Necessity

Hello everybody,

I was wondering if anyone can tell me what needs to be in the PCP letter for insurance purposes. I want to make sure it's appropriate for approval the first go round. Also, my PCP has some reservations about the sleeve, but not the band. She keeps trying to talk me into the band, so I've just started mentioning that I'm planning to forward with weight loss surgery, and not mention what type specifically. Can the letter say just weight loss surgery, or does it have to say what type of surgery specifically?

it helps if the letter is specific' date=' since the insurance company wants you to play by their rules.

Thanks, Ben. My PCP is a little Asian lady, who probably wears a size 0-1. She thinks the sleeve is too drastic, but she refills my bp and diabetes meds every 3 mos. If that doesn't signify that I need something drastic, I don't know what does.

My surgeon's office gave us a "Model" letter for our PCP to use. He ( I think it might be a language barrier thing) wrote on my letter gastric bypass.

I have my physical due this month. I have photocopied his letter, making the changes for the sleeve.

Since my last nut meeting is this month, I want everything in order, pcp letter, too.

      April Surgery
      Am I the only struggling to get weight down. I started with weight of 297 and now im 280 but seem to not lose more weight. My nutrtionist told me not to worry about the pounds because I might still be losing inches. However, I do not really see much of a difference is this happen to any of you, if so any tips?

      Well recovering from gallbladder removal was a lot like recovering from the modified duodenal switch surgery, twice in 4 months yay 🥳😭. I'm having to battle cravings for everything i shouldn't have, on top of trying to figure out what happens after i eat something. Sigh, let me fast forward a couple of months when everyday isn't a constant battle and i can function like a normal person again! 😞
