Super Sleeve (Gastric Plication)

OK, so I'm reading all the super stories here and I'm 80% sold on a procedure that six months ago I would have told you "no way!" and while doing my research, as so many of you say to do, I ran across this LINK: http://www.promedica.com.gr/erevna_en.htm

What is super sleeve?

_supersleeve.jpgLaparoscopic sleeve gastrectomy is not so simple as any other gastrectomy. There are some important technical details, unknown to inexperienced surgeons. Gastric tube size influences both the degree of weight loss and weight stability. A large sleeve predisposes to gastric dilatation and weight regain. Inexperienced surgeons tend to create large tubes (:thumbup1: or to leave back large remnants of the gastric fundus. A standardised technique with a gastric capacity less than 100 mL (A) is mandatory in order to get the patient achieve a durable weight loss. The sleeve should be "super" which means a small gastric sleeve diameter and a higher degree of restriction. In our hands, with the gastric capacity been restricted to 60-80 gr and Ghrelin (the appetite hormone) totally suppressed, the resultant weight loss is predictable and comparable to this achieved with gastric by-pass (60-70% EWL), without any serious complication or side effects (e.g. Vitamin malabsorption). We introduced super sleeve in our bariatric program, as the most effective and safe bariatric solution even for the super-obese patients.

There is one that has had the procedure, and she's posted her story.

I think she had a bad band, and didn't want to go as drastic with sleeve.

Personally, I'm not convinced. I'd like to see ghrelin level tests similar to what they've done with RNY and Band vs. Sleeve ghrelin levels. It states Ghrelin is "suppressed", but I'm curious if it's suppressed by satiety achieved quicker due to the restriction, and how long does it last. Since the fundus is left intact, the tissue is alive, and producing Ghrelin. A lot of band patients say they are never hungry like they were before surgery. So, I'm interested to see actual ghrelin plasma levels at 3, 6, 12, 24 and 36 months.

My comfort with the VSG was the fact it's stage 1 of the DS which has been done forever. Plus, gastrectomies in general for cancer and ulcer patients.

I don't see anything super about it. Just me personally, I think it's a great option if that is what someone wants, but I went into weight loss surgery for the long haul. Not with the mindset of having something reversed.

I agree with what you've said. To me, the only benefit is a short-lived one: no leaks from staple lines. Of course that's also why I'm looking for the most experienced surgeon I can find, so that my surgery is simply routine. Thanks.

According to this Super sleeve and Sleeve plication are two different things. This is saying "super sleeve" is more retrictive partial gastrectomy--It may just be a marketing spin on utilizing a smaller bougie (30-36) like most docs have done for over 3 years now...not sure but it is saying in the description there that more of the fundus is removed reducing ghrelin completely--With plication there is no ghrelin removal.

Just confirmed they are 2 different proceedures done by the same surgical group. The so-called "super sleeve" Is a more restrictive VSG under 100ml capacity, but it is also a Partial gastrectomy (partial stomach removal). The same surgery group does a vertical plication (stomach folding) as well but it is a different option.

To clarify: The plication is not the super sleeve. The super sleeve is a more restrictive VSG.

Just confirmed they are 2 different proceedures done by the same surgical group. The so-called "super sleeve" Is a more restrictive VSG under 100ml capacity, but it is also a Partial gastrectomy (partial stomach removal). The same surgery group does a vertical plication (stomach folding) as well but it is a different option.

To clarify: The plication is not the super sleeve. The super sleeve is a more restrictive VSG.

I am talking to BeLiteWeight.com about having a Super Sleeve done in at Star Medica Hospital, 15 mins across the border from El Paso, Texas in Juarez, Mexico by Dr. Jose Rodriguez. The cost is $6200 for everything.

Has anyone gotten the super sleeve done or know of anyone that has. OR Has anyone worked with this company or Doctor or even hospital?

Can I trust them?

I would ask Belite for some email addresses of patients that have had the super sleeve. They would be the best ones to answer your questions.

I actually had the super sleeve in Tuijana Mexico, by Dr Jose Rodriguez..I orginally was looking into getting the Lap Band and did my research for that and actually went to get the lap band, but it was changed at the last minute to the "Super Sleeve" I have had no complications. The Super Sleeve sounds alot scarier than it actually is. With the lap band I had to worry about slippage and what I ate. This super sleeve that I got, I can basically eat what ever I want. I just get full really fast I can't even eat a whole kids meal form Mcdonalds. (I eat like 2 chicken tenders and feel like I ate a thanksgiving dinner)The super sleeve from what I understand they did is, Fold my stomach inside of itself and Basically double stitch it. So they did'nt cut my stomach or anything like they do in a gastric bypass. I have 4 tiny incisions on my stomach that are fading. If you want to see the incisions I'd be willing to take a picture and forward it to you. If you have any questions let me know..

I 100% I got the super sleeve done 7-12-10 and I love it. My sister has a lot of issues with her lap band that I do not with my super sleeve. What sold me was the fact that they do not cut my stomach. The whole idea of WLS is scary and that just helped with my fears. And if you are worried you won?t lose as much weight think again!       Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
