Yes I have it. I needed to show medical history for 5 yrs. had to do the nutrition class and the phyc. evaluation. I had my 1st consult with surgeon on Jan 4,2018 and had my surgery done on March 22,2013. Approval went very fast once everything required was turned in.

I have it also, I had to do 1 year with all drs, then I was denied at 1st but I called and spoke to them and my dr sent a letter and it was approved...

I started in July, not really sure if I wanted to proceed with any of the surgeries. I had to do a psych eval, endoscopy, 3 weigh-ins (30 days apart), 2 nutrition classes and then submit for approval. Took 6 days for approval. Scheduled for end of November. Since I wasn't necessarily in a hurry to get it done, I wasn't worried about when appointments were etc. I think I could of realistically had it all completed in 3 months.
