Interested in the Sleeve, Dr. recommended Bypass

Hey, everyone!

I went to the PI seminar on, June 15th, had my Psych test and met with the Dietician on, July 15th, then had my first appointment with the Dr. on, July 29th. I have been looking into the Sleeve. I have always been kind of afraid, if you will, of the Bypass because of Nutrient Deficiencies, rerouting the intestines....etc. When I met with Dr. Nguyen, he said, given that I have irregular periods, PCOS, and infertility he believes I also probably have a Metabolic issue. Which would be why I can eat clean, and kill it in the gym with minimal results. He recommended that I consider the Bypass, instead of the Sleeve. He said he doesn't want me to sell myself short, by getting something that may not give me maximum weight loss results. Has anyone else had their Doctor tell them this? If so, which surgery did you go with? How did you do with it?

Ok I know many ppl that have had the sleeve and a couple that had bypass all are happy with results that being said every WLS has suggested bypass to all of us even pushing it in some cases the heavier you are the more they push its YOUR decision not theirs so far there isn't a lot of long term research on the sleeve but many feel the results are pretty similar I started out very heavy over 400 I lost 60 lbs pre op on south beach Diet as required by surgeon to lose at least 5% before surgery which I had at end of February this year and I'm down 105 lbs since then I've had no problems

I have a band and I am seeing Dr Goldstein about a revision to a sleeve.I am afraid of a bypass.I find it interesting that your doctor recommended a bypass.I have had PCOS for years.The band is a complete failure."Bypass??

Didn't Carnie Wilson have the bypass?

Shellyd88 did you have the sleeve or bypass? I agree, it is my decision. I weigh 229 and can't drop below 219 or I won't be approved. I have like 100lbs to lose. Thank you both for your replies!

I weigh 226 and my Dr suggested the sleeve for me... I have horribly irregular periods, hypothyroid, and I'm insulin resistant. Did he say why exactly he's choosing bypass over the sleeve?

@Rdepps Based on your posting, I would DEFINITELY get a second opinion. There is a lot of variability among surgeons and NUTs on pre-op and post-op care as well as which surgery is most appropriate. Bottom line is that it is your body and you must live with the results. Best of luck to you!

Glitterpockets he said he would choose Bypass over sleeve for me because the Sleeve doesn't fix Metabolic issues. I really need to clarify but I think it has something to do w an enzyme that is produced. The bypass removes the nerve or something that causes the release of the enzyme.

I think the hormone is also suppressed with the sleeve as well.

I have PCOS, insulin resistant, and metabolic issues, as well as high blood pressure, and my Dr recommended the sleeve. Like others said, I'd get a second opinion. With either one you choose, I wish you much success, and the same to everyone else!

The enzyme grehlin is also removed with the sleeve. I had diabetes, HBP and high cholesterol. My surgeon listened to my opinions on why I wanted the sleeve and agreed that it was the best option for me. I came off my blood sugar meds the day of surgery and my other meds have been cut in half so far.

As I understand it the weight loss is a bit slower with the sleeve compared to the bypass but we all end up at goal if we stick to the plan. Do what is right for you, this is a major surgery and you should be happy with it because it is your life and you will have to live with it forever.

I was pre-diabetic and am PCOS and my surgeon and my endocrinologist both recommended the sleeve. I am now 6 months out. I started at 242 ten days before surgery. I lost 12 pounds on the pre-op so was 230 on the day of surgery. I am now 174 this morning. I don't get nearly the hunger I used to get.

I would get a second opinion from a surgeon and speak to your endocrinologist. If your endocrinologist isn't familiar with the different WLS surgeries then either find one that is or see if your endocrinologist is willing to educate themselves about the various surgeries. You could also have the potential surgeon talk to your endocrinologist. The surgeon can then discuss from a hormonal POV why her prefers one over the other and your endocrinologist can decide whether that is sound medicine or personal bias.

If you don't have an endocrinologist for your PCOS, IMO, you should. I think they are best suited for dealing with PCOS and most metabolic issues. A fertility specialist OB/GYN can be good if your goal is getting pregnant but from an overall health perspective I think it is important to have an endocrinologist.

I've been researching for a little big time on this and have 3 surgeons go through the pro/cons of each.

The sleeve- you tend to lose slower & less overall as a norm. It's a 50/50 shot with a plateau/ wall that many experience and I've read on here. There are a lot of pros and until last week it was my number 1 choice.

I've been digging deeper & deeper ( and I love to research) and although bypass seems more invasive in the long run its not. You do lose more and faster as the norm! I've had to honestly sit and look at the statistics carefully and weigh each. I've read so much I know I'm informed well to make my own choice but I'd talk to others doctors to get another opinion

A bariatric seminar is a good thing to go to if they have them in any of you alls area, I went to one tonight, the speakers were the bariatric Dr, and surgeon, it was very informative, and answered a lot of my questions.

@@Rdepps I had the sleeve I've lost 107 lbs since surgery at end of February this year and 60 before that

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL
