Not losing a great deal of weight on the pre-op diet

Been on holiday and have gained 3kg, so decided to start my pre-op diet early (doing it for 3 weeks instead of the required 2 weeks) and it's good job I did as I am currently on day 8 of the pre-op (Liver reducing) diet and only managed to lose 1kg. I've followed the diet rigidly even being more strict than the actual diet states! I've managed (just) to get my required 2 litres of Water plus my3/4 cups of tea (with milk from 200ml allowance for Cereal and tea) per day. Called the hospital Specialist Bariatric Dietitian earlier today to discuss my concerns as I am really worried that my surgery will be cancelled because I haven't lost enough weight. The SBD went through everything I had eaten in the past 8 day and he said I was doing everything right but I could do with taking in extra veggies and try to do a little more walking and that maybe I have bit of water retention (not cycle-related) as I have on-going problems with painful swollen legs. Am to give him a call again next week if I'm still worried.

The only solution I can think of is to completely cut out ALL carbs, starch and dairy (except cheese) and just eat fruit and Protein foods only.

Anyone else have/had this problem? Can anyone give me some advice?

Why cheese? If you are going to eat dairy, IMO yogurt is a healthier choice. And why fruit instead of green veggies? Green veggies are much lower carb.

All that said, losing 1 kg in a week is great IMO. Does your bariatric team have a specific goal that you must meet? It sounds like it, from your post, but personally I think expecting more than 1 kg/week is a high and kind of cruel standard.

Hi, I'm in the uk too. Despite my dietician telling me I had to eat the carbs on my pre op diet, I didn't as I knew that would severely limit my weight loss. So I cut out the carbs and substituted with green veggies and a little more lean Protein and lost 12lb in the two weeks. Dietician and surgeon were very pleased, and I just kept it to myself that I'd modified things a bit! I have pcos and I know my body and carbs are my biggest problem in preventing weight loss. The one size fits all approach from dietitians in the uk (low fat and whole grain carbs) does not work for everyone!

cheese livens up Spinach (1 piece matchbox size, once a week) and I'm not intending to just eat fruit instead of green veggies, I intend to eat BOTH - some fruit and lots of (non-starchy) green veggies and salads and high Protein food.

I wasn't given a specific goal per se, but was told that it is possible that you will lose a lot of weight on this diet, but it will mainly be Water loss. When I talked to the SBDietitian he said that he had just seen another woman that morning who had lost 7kg in her first week so my lack t loss weight loss was probably due to Water retention but to call him again next week if I still haven't had a great weight loss - great help, not.

And here's what whizzing around my head at the mo - if I can't lose weight on the pre-op diet, what hope is there for me after the surgery in meeting hospital weight loss goals?

Thank you, Madferitchick for the feedback - will give it another week then I think I will follow your lead. You're so right about UK dietitians, the one size fits all approach doesn't work for everyone - a good example of that statement is ME!

Thanks for asking. Weight loss not great, only managed to lose 2kg over a 2 week period. So this weekend I'm going to try @@madferitchick approach and hope I get better results.

Hi @@AJ cowgirl, only lost 4kg on the pre-op diet, the surgeon would have liked me to have lost more but was happy to go a head with the procedure.

I'm now 2 weeks post-op and feeling great. I posted a 'thank you' message for everyone's support and advice and must of missed you off the 'Mention' list - so sorry.

Anyways, the surgery went well. Now 2 weeks into the liquid diet. Stitches have healed nicely. Following all instructions regarding meds from the Bariatric Team. Everything seems to be going according to plan. Thanks for keeping in touch and for your support.

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL

      Still purging all of the larger clothing. This morning, a shirt that I ADORED wearing ended up on top. Hard to let it go, but it was also hard to let go of those habits that also no longer serve my highest good. Onward and upward!
