How did everyone go about telling manager you're having surgery?

I was approved this week. .and schedule surgery for 4/15..is that adequate time to give job notice. .it's 3wks? Also how did you guys go about telling your manager. I applied for FMLA on Tue. Doctor said I will have to take 4 wks off. I work in a hospital do some patient pulling. I know FMLA will send information to my director. but I never see her..we have a department manger I see daily. .how should I tell her? Really hope she do not ask me, what type of surgery I'm having. Cause I'm a private person and I don't need coworkers knowing my personal affairs. .that's also why I opted to have surgery at a different hospital. Even if it will cause me extra. My department is judgemental. .I would think as nurses they would be more understanding about how unhealthy it is to be obese. .Plus I hear how they bad mouth bariatric patients that comes to our unit post op..smh
