There's lots of reasons, not losing the amount of weight they ask you to lose before surgery, gaining weight. Not passing the psych evaluation, losing too much weight and being under the BMI you need to have. Make sure you know everything they need you do Have/do before starting. Call the insurance co and ask them for every detail so there won't be any surprises. Don't be afraid to call them!

Whatever dr your using can guide you along they deal with insurance all the time they know what you need to do

I agree with Shelly, ask your doctor if they are familiar with your insurance companies requirements. They should know.

But also do your homework, call your insurance company. If you have a pre Authorization number on your card, call that one. They tend to know more about your specific plan than the customer service rep.

make sure you call your insurance company and ask to speak with the bariatric cordinater thats how i got the ball rolling if they dont have one of those ask to speak with the prior authorirasation hotline they will know your requirements
