How strict are they about the two year weight history?

I don't have one and I am afraid that I will be denied.

I didn't have a two year weight history either. I got Aetna last August and attained a new doctor in September. I started seeing her for weight management back in February and once every month there after. My bariatric surgeon's patient coordinator sent all my paper work to Aetna and I received approval within 4 days.

Did you have 4 nutrition visits or just one nutrition visit and 4 visits with surgeon?

4 nut visits. U can look up requirements. Is it policy 0157. Search for that it comes up.

I got a total of 4 because the first visit was not considered one because it was my first appointment

I had 1 surgeon visit, 3 nutrition, 5 General practitioner, 1 psych.

I started seeing my GP in Feb so that it was over the amount required just in case.

Finally approved yesterday! 4 surgeon visits, 1 nutrition eval 1 psych eval, other tests and labs required by surgeon
