I have Horizon BCBS. I have to submit a letter to the insurance company regarding my attempts to lose weight over the past 12 months. I'm curious how detailed this letter should be. I don't want to say too much or not enough that might get me denied. As anyone had to do this? If so, can you provide some insight on what I should write?
