Hi guys. Does anyone have united healthcare community plan? Every time I call them up and ask about requirements, there's never a clear answer! Any help would be much appreciated =)

I do not have the community plan but have you tried looking on uhc.com and checking under your plan to see if the bariatric surgeon is under network. United Healthcare was also very quick to come back with approval. I hope this helps a little if at all.

It can be different for each employer. Because of my employer my requirements were quite a bit different than the standard.

You need to get access (through HR or UHC) to your benefits and coverage guide.
