Stanford Hospital And Consultation Question

I just got a letter of approval from my insurance company for a consultation appt at Stanford. My doctor told me that after my consultation appt I will find out if I get approval for the surgery. What happens at the consultation appt? My doctor told me that my BMI is not high enough but I have several morbid tendencies. I have already gone through 6 months working with a nutritionist and 6 months on Weight Watchers.
