I hate for anyone to have to pay $50 for it, when I bought it and can share the information!!!

If I were you, I'd contact obesitylaw.com. They're advocates to help people fight for approval. Kelly Lindstrom helped me, they're super nice and they know how to navigate through the insurance bs. They file your appeals for you.
