Menu -   Protein shake, made with 1/4 cup oatmeal/2 strawberries/skim milk/1/4 banana - its feeling and gives me so much energy - I added up the calories for the shake 1 cup skim milk - 80 cal 9g Protein 2 strawberries - 8 cal - 0 protein 1/4 of a banana - 23 cal - 0 protein 1 scoop protein powder - 120 cal - 23g protein ______________________________ Total cal 231 total protein - 32   I think that is a pretty good amount of protein for bfast! And not bad on calaories, and the fact it feels me up until lunch, and gives me energy, it beats the coffee with cream   Lunch was on the run, so swung thru starbucks and got the carmel light frappacino - 220 calories   Snack string cheese - 80 calories   Dinner 2 oz of chicken breast - 96 salad - 225 salsa as my dressing with just a squeeze of ranch - 50   Total Calories for salad- 371   Total calories for the day - 902   I probably had a bit more calories because I did have a bite of granola bar, and a few bites of my dd sandwhich. So, maybe I had 1000 or 1050.   Exerice - Oh yeah, I worked it!   16 min on Elliptical Upper body workout - Triceps (they are already hurtin) Biceps, shoulders, back, chest Elliptical Machine - 10 min - I was planning on doing about 25 min, however, after about 10 min my knee started popping so I got off, I didn't want it to give out on me and I go down like a bag of bricks. I knew if that happened, I would have a major set back, so I decided that was my clue to get off the machine.

I had a busy weekend, Saturday was the special olympics that I worked. I worked the softball throw and I was running all over the place keeping up with the special needs kids. I tell you, they may have limited mental abilities, or physical abilities, but they are stars in my eyes. They had so much energy! I was exhausted by the time the day was over!   Well, my fill is now again, loose! I just don't understand it. I'm very frustrated, but I will not give up. I just don't know why I'm so different.   DH & I went out for steak Friday night,I could have eaten more, but I stopped at about 2 to 3 oz of the steak. I also had turkey last night, and decided to "test" a roll, it was small yeast roll and it went down as smooth as ever, I didn't eat any more, but I know now the PB incident was just a fluke on the toast.   I am going to call the doctor and discuss it with him. I'm not going to let this be an excuse to "eat" and I'm still hoping at about 2 or 3 weeks the fill will really kick in. It has done that in the past, so I'm going to discuss it with the doc and see what he thinks.   Menu today:   Bfast eggs/ 1 sausage link   Lunch - salad   Dinner - roasted chicken and salad   Exercise - 15 min on treadmill - 107 calories .75 distance   Exercise PM - Lower body workout - really worked the abs & legs, then jumped on the elliptical for 10 more minutes of pure sweat!   Wt - 180.7 - grrrrrr - But TOM is here, has been since last Friday, so he will be leaving soon and I should start dropping again - at least I hope!

4/11/08 - The fill is working!

Okay, I am sure the fill is working and I don't think I will be going back.   I noticed last night when I was eating my dinner, it sure didn't take much to fill me up. Then, this morning was our office breakfast and it was my turn to pick it up. So, we all decided Steak & Shake, so I ordered me the cheese scrambler with wheat toast.   Okay, I take one bite of the eggs, and decided I would eat 1/2 slice of the wheat toast. Well, I've always been able to eat breat in the past, and plus this was toated, so no biggie right? Well..........wrong - it was long and I was running to the bathroom PBing. Wow, I actually PB'D! I'm excited that I have enough restriction now that I actually PB'd! After 15 1/2 months, I've got real restriction! Thank you thank you thank you!   I'm on cloud 9! No more bread for me! I finally have found a food that I cannot eat! Yay!   Menu today   Bfast - eggs/part of bread (whatever I didn't PB up)   Lunch Protein shake (my stomach hurts a bit from yacking so much, I think I will keep it simple)   Dinner - we're going out tonight, so not sure, but it will be healthy   Exercise - LBWO - I didn't make it to the gym last night to do the lower Body workout, so I will do it today.   Wt - still have not weighed, but will on Monday! TOM is around the corner, so the scales might not be nice to me on Monday, but I will still weigh on Monday.

I had a GREAT workout last night, my DH & I went to the gym and I worked my upper body out. I worked it so hard, I felt like mush when I left. But it sure felt good to be back in the gym workout out like i used to do! I'm excited to be back in the program! I can't wait for 1 month from now, because I know if I am consistant, the results will start showing.   Menu today   Bfast 1/2 cup skim milk, grapenuts cereal (about 1/2 cup)   Snack yogurt   Lunch tomatoe soup (this is the last of my container, thank gawd, getting tired of it, but it is good)   snack - handful of nuts   Dinner 1/2 cup of beans & ham - yum   Exercise 10:00 am - 15 min on elliptical   2:30 pm - 22 min of HIIT - burned 181 calories, and 1.71 distance   Gym - my plan is go tonight and work my lower body - the abs are screaming for crunches!

NOt sure about this fill

Okay - I just knew this was going to be the right fill. I do seem to get full quicker. However, I just don't feel it? I know, I should just be happy that I eat less.   In the past it has taken a few weeks for my fill to really kick in, so I'm hoping that will happen.   I'm very hopeful it will work, but I'm also scared this fill doesn't work. I'm running out of room for any further fills.   So, I'm not going to give up.   I had weight wise oatmeal this morning, only ate about 1/2 of it.   Lunch Tomatoe soup   Dinner Protein shake, maybe some smashed up beans I made too.   Exercise - 10:00 am 15 min on elliptical machine 3:00 pm - 15 min on elliptical machine

Fill History

Fill #1 - 3/6/07 - 2.2cc's   Fill #2 - 4/17/07 - 3.0cc's   Fill #3 - 7/6/07 - 3.5 cc's (I think it was only 2.5cc's because when I went back for fill #4, he could only w/draw 1.8 cc's)   Fill #4 - 7/20/07 - filled me back to 3.0 ccs - He should have filled me more!   Fill #5 - 8/17/07 - 3.4cc's   Fill #6 - 10/08/07 - filled me to 3.8cc's - the restriction I've ever had,but still could eat big bites of food, so I figured I needed to be tighter.   Fill #7 - 11/21/07 filled to 4.0 cc's. By the evening could not hold anything down, not even my saliva. PBing all night - horrilble experieince   11/23/07 - Unfill - Got unfill and because of the swelling and all the PBing, doc took my fill down to 3.0cc's. NO RESTRICTION AGAIN   Fill #8 - 1/18/08 Went in and the doc filled me back up to just right under 4.0, according to Flouro, it was perfect. Well, by next morning, could not hold down saliva again, ended up back in hospital and the Doc had to take me back down to 3.0. grrrr. Now I'm back where I was in November, barely restriction again.   Fill #9 - 3.5 cc's - was tight for about a week, now feel lose again. grrrr, still have "some" restriction, just not quite enough! Next fill scheduled for 4/7/08 - I hate that it takes sooooooo long to get in to see the fill doc.   FILL #10 - 4/7/08 The doc just added .4cc's, making me at 3.9cc's. I think this might be the one!

Fill #10 - I think I'm finally where I need to be!

Okay - I just came from fill #10. The doc did something different, he usually takes all my fill out and then refills it. But since I've had so many fills, he just added to it. I was at 3.5 cc, he filled me .4cc, which gives me 3.9 cc's. I remember the last time I had pretty good restriction was at 3.8 cc, then he filled me to 4.0 cc's and all my troubles began.   All along, I've always thought 3.9 cc's would be the perfect spot. Well, finally after 15 months I'm at my sweet spot - or at least I think I am. Its still too early to tell. I will be on liquids all day today and possibly tomorrow, then I go on mushies for 2 days, then to regular food.   He had me eat some jello while I was there, I can still feel it in my pouch, I ate about 1/2 of the container, and I was stuffed. Now, the problem is, I'm always tighter in the am and loose in the evening, so tonight will be the real test.   Menu today   bfast - jello   Lunch - tomatoe soup - puree   Dinner - protein shake   Exercise - 3:00 pm - 15 min on elliptical machine

Today I'm doing something I know I shouldn't - I'm just flying by on my menu. When you don't plan you plan to fail, as the old saying goes. I need to be careful with what I choose to eat today.   Here is my plan:   Bfast QT coffee, iced   Snack string cheese   Lunch roasted chicken salad (going out to run errands and I will run by a local deli and pick this up)   Dinner - ground hamburger, small orange - weird combo I know, but I'm thinking this is what I will have   10:00 am - 15 min on elliptcial machine 3:00 pm - 15 min on elliptical machine   9:30 - LBWO at the gym   11:00 pm - be in bed and asleep - I know this probably WON'T happen, but thats my plan   My fill is next week, I'm SOOOOO ready for it!   Wt - Another day I didn't weigh! Yay!

Trying not to be down on myself

You know, I know I can't change the past, I know I've come a long way since last year, but sometimes I am just a little disappointed - not in the band, but in myself.   It all began with my cruise last year, after I fell, and had to have surgery, along with my house flooding when we returned home, and my grandmother dying, everything went to H-E - _ _.   I ate whatever I wanted, I couldn't exercise, I was so depressed and just ate. The habits came rushing back in full swing. I'm disappointed in myself because I thought I had changed. Now I look at me 14 months post op and I haven't lost weight since - well, November I think I weighed my lowest, 167. I did so good the first 6 months, but the 2nd 6 months were pure aweful!   I know, there is nothing I can do about it now, I'm just venting. I am now determined to get 'er done! I want to reach or be pretty close to my goal by my 18 month post op anniversary.   I just had to write down my frustration and my disappointment in myself.   Now, I'm ready to kick some butt and ready to lose this weight. I think my mind is finally right!

How to get insurance to pay for your PS part 2

Another important thing is to be diligent. Insurance companies often have tactics to discourage you from seeking approval for procedures. Usually, the insurance company would prefer that you pay for reconstructive procedures yourself. So if you are initially denied: appeal, appeal, appeal. By law, the steps detailing how to appeal are listed on any letter that informs you of a denial. Take advantage of the information, and be prepared to submit an appeal in writing as necessary. Although with insurance companies, written documentation is king. But don’t be afraid to also call the insurance company to try to find out what factors led to their decision. (That’s what I did after I was initially denied, once I found out who made the decision, I called more than once and asked if my entire file was sent. After some research, I had the file sent again to the doctor who acted as the clinical director and I was later approved. ) Once documentation is received by the insurance company you'd be surprised how often file documents are separated, lost or mislaid. So if the insurance company or a specific department of the company (like the Pre-Determination department, which often makes the decision for reconstructive surgeries) says they didn’t receive the photos along with your file - then ask your surgeon’s office send them again. If they don’t have copies of letters from your other doctors, be prepared to send those again as well. Make sure you are fully prepared with any documentation they would need to make their decision, and you are better assured of approval. NOTES - I don't really know who to give credit for, but I found it while googling on someones blog, I couldn't find a name. But It is not mine - although this person was very informative and I appreciate her cander.

How to get insurance to pay for your PS

I found this while searching on the net, so I wanted to save it for future reference. I'm down around 70 to 80 lbs and so far the only thing I'm experiencing is lower back pain. I still have more weight to lose, but so far everyone tells me no need for tummy tuck. But you never know, I want to have all the info I can in case I do require one.     GET INSURANCE APPROVAL FOR COSMETIC SURGERY   Since I previously worked at an insurance company and have discussed what I’ve learned there, I often get inquiries about how to get insurance approval for cosmetic surgery after weight loss. It is possible to have the insurance company approve payment for your reconstructive surgery. The most common type of surgery requested after weight loss is an abdominoplasty. This single surgery can make one of the biggest differences in the body – it removes excess skin and tissue of the abdomen area. This procedure is also often called a “Tummy Tuck.” Do not EVER use the term Tummy Tuck when communicating with your insurance company – they consider this strictly a cosmetic surgery term and they do not like it. Once thing I’ve learned with having worked at an insurance company is that how you communicate with them is as important as what you say. The abdominoplasty is a term used to cover a few different procedures. Two of the terms, panniculectomy and abdominoplasty are often used interchangeably, but the terms actually have some subtle differences.   In a panniculectomy, the hanging or excess skin of the pannus (the lower part of your abdomen) is removed surgically. In this procedure, muscle repair may not be done – in most adults who have had weight changes or pregnancy there is often some separation of the abdominal muscles.   In an abdominoplasty, this excess skin is removed and often there is a repair of the abdominal muscles if there is separation or loosening of the muscles in this area. Hernias in this area are usually covered by insurance, and the repair of these hernias is often done simultaneously with an abdominoplasty.   I think the keys to insurance approval are very simple. It's difficult for one doctor to completely ignore another - so I always try to have my insurance approvals passed on to the clinical doctor in whatever group is in charge of making the decision.   To make sure this happens, I include a lot of doctors' letters in any file that is going to be sent to an insurance company. By this I mean letters from your PCP or General Practitioner, letters from your OB/Gyn, your dermatologist - any doctors you have.Add these letters along with the file and photos that your Plastic Surgeon will send to the insurance company.   Now after a massive weight loss, it is inevitable you'll have lots of excess skin. For that reason, I suggest you make sure that the insurance company understands that it's not cosmetic reasons that you want this skin removed - but for medical reasons.   In other words, you want to make sure that they understand that this excess skin (and excess tissue, especially in the areas of your abdomen, thighs, hips etc - and perhaps even your bust) is interfering with your daily function and life.   The things I would stress would be the issues that occur with excess skin. These may seem gross but they are actual medicalresults of excess or hanging skin. If your hanging abdominal skin is forming an apron of skin that hangs toward your pelvic area, then this may cause complications that can be used as justification for approving your abdominoplasty surgery.   The idea is if you have any of these conditions to make sure that your doctor notes you've been diagnosed or treated with it. Or if you haven't been diagnosed but the doctor knows you are PRONE to this conditions this can be used as justification for getting your surgery before you suffer from these conditions. For example, if a female has been treated for yeast infections from her OB/Gyn, then it is perfectly acceptable for that doctor to note that the hanging skin might make you prone to that condition (even if it has not yet surfaced.)   Letters from your dermatologist noting skin irritations, rashes, infections from your hanging skin or having been treated with medication for these conditions is also a very strong factor in getting approval. A letter from your urologist might mention if you've been diagnosed as having excess abdominal skin that is pressing or your bladder or urethra. This pressure can cause various urological conditions and disorders.   If you’ve been treated by a chiropractor for any lower back pain, any type of alignment difficulties or other conditions that might be affected by hanging or excess skin, get a letter documenting that.   It has been studied that every one pound of abdominal tissue add 10 pounds of strain on the disks of the spine and lower back. The extra weight affects the normal curve of the spine and contributes to lower back pain. In other words, an extra five pounds can add 50 lbs of strain to the lower back. And extra ten pounds of abdominal tissue can add 100 lbs of strain to the disks of the lower back and this is another reason this area common suffers abdominal hernias (this is especially true after changes in weight, pregnancy, and abdominal surgeries).   Your PCP or Family doctor can also be of great help in helping the insurance company to realize that getting an abdominoplasty is reconstructive and not cosmetic. Having this doctor detail the conditions you’ve experienced is essential to getting approval for this procedure.   Insurance companies are often in the business of not approving procedures that they even suspect are not medically necessary but cosmetic. Make sure that your documentation reflects common medical industry terminology (writing panniculectomy or abdominoplasty instead of Tummy Tuck, as discussed earlier) and utilizing the correct codes to document the conditions that you may suffer from. If you have even more significant excess skin that extends throughout your lower body area, then the more severe Belt Lipectomy or even a Lower Body Lift may be needed. This procedure involves a full around-the-world incision that spans your entire body. The surgical and recovery times are also more lengthy in this much more costly procedure and often insurance companies are hesitant to approve a Belt Lipectomy or Lower Body Lift without some significant documentation to show that it is needed.   The CPT code for the abdominoplasty procedure is generally CPT 15831 – The excision of excessive skin and subcutaneous tissue. (This is the code that is on my insurance approval letter for my abdominoplasty.) The code for removal of excess hip area tissue is CPT 15834. With other areas included (flanks, back) it is CPT 15839. If this process includes suction-assisted lipectomy (like liposuction) then it can be CPT 15877. (Note: Insurance companies are notoriously loathe to approve CPT 15877 as they consider it cosmetic.)   It is important to document that the excess skin is not a cosmetic issue but is “interfering with exercise, hygiene and health” and that it is not cosmetic.   The following is a list of common industry ICD-9 codes are descriptive diagnostic codes that used to describe symptoms, in this case relating to excess skin. These can be used in the letters that describe the various conditions and symptoms that are prompting you to seek the surgery.   Panniculitis 729.39 - Skin irritation underneath pannus area Large pannus 701.9 - Redundant skin, lower abdominal area Intertrigo 695.89 - Rashes & Irritation of skin in pannus area Back pain 724.1 - Back pain from diminished abdominal muscle Skin Abcess 682.2 - Can result from excess or hanging skin   This is just some of the information that can be included in the documentation provided to the insurance company. I know I did not want to wait until my excess skin became a full hanging apron, so I knew I had to be persistent and diligent in seeking insurance approval.

What happened here, let me explain myself, I had a single friend of ours over for dinner because I made too much food. (I always think about the single men since they don't have a wife at home cooking for them), anyhow, he felt obligated to bring a dessert - even though I told him not to, he still brought a cheesecake factory cheesecake. So, I felt obligated to have a slice. I should have just eaten 1/2 slice - but no, I ate the whole darned slice.   Anyhow, after eating the cheesecake, I headed to the gym and worked off 200 calories.   Scale in the am stated I weighed 183 - arggg, sucks~   I will see the number go down, I will, I will. I'm determined! I'm not giving up, I must see the 160 again, soon! I have 100 days or so left until my cruise, I refuse to go weighing what I do now. I want to look halfway decent in my bathing suit you know.   Well, I will start checking in more on my journal and updating my menu. This seems to keep me on track better.

EAting has been okay, not too bad, but not 100% perfect either. My exercise has been BAD. I am planning on making it to the gym tonight, so we'll see.   I didn't weigh, I was supposed to weigh this morning, but completly forgot. I was running late this morning to work, so just didn't think about it.   Will update later on whether I made it to the gym or not.

staying on my plan today, I have done pretty good with my eating this week, however, my exercise SUCKS.   I've had so much going on, my remodel of my house is ongoing, almost finished but not quite. My mother went into the hospital last night for pneumonia and she is being kept because her EKG showed she had a mild heart attack at some point. Then, my mother in law was also taken to the hospital for chest pains and she is having a heart cath done right now. Needless to say, with all the hospital visits, my eating has not been planned, but I've eaten very little. I feel a little more restriction now, must be stress.

I hate getting old, I swear, I feel like my body is falling apart. My back hurts so bad, my kneck, my knee, grrrrrr. I want to have the energy I used to have. It wears me out just to clean my house anymore. I know the more weight I lose, the better I will feel. That should motivate me to continue my journey.   Menu today   Bfast 2 boiled eggs, 2 thin bacons with mustard & mayo mixed in together. Made a bacon/egg salad. good   Mid-morning coffee w/cream   Lunch tuna - mustard celery stick   Dinner grilled
