Getting approved was easy! I am just on the line of meeting the BMI requirements and have no co morbidities. I will say a special prayer for you. Keep me posted.

Quick question - did you have to pay out-of-network fees? My surgeon is listed as a Horizon provider, so I am hoping this would be considered in-network ($400 out-of-pocket limit), but I'm guessing that's too much to hope for!!
