APPROVED by Horizon Blue Cross Blue Shield on the first try! Getting sleeved on 9/21/2011!Not yet!Thanks for responding! How many visits did you do to your primary care doctor? I was told on the phone to go two or three times? Did you loose any weight before your approval? If I do my BMI will go below 40. Not sure what to do.Awesome. I also have horizon bcbs and am wondering, how long did your approval take? and how long was your supervised diet?. The policy is not clear as to how long you must do the supervised diet. My Dr. is thinking 3 months and then submit. Any information would be appreciated. Thanks!Has anyone been approved by Horizon BC/BS since there policy changed and how long of a supervised diet did you do?
