I myself did self pay for $5000 plus another $500 or so for travel and lodging. I did not pursue insurance because I did not want to wait for approval, my health was declining fast.

What?? Where did you go? My UHC co-pay in network is $5000.

$5K total.

My insurance had a $750 deductible and $750 out of pocket then it covered 100%. My deductible was meet with prior doctor's visits, sleep study. I met the out of pocket with the surgery. This year the deductible increased to $1700.

$0 - I had already hit my $5k deductible doing other things last year..heck, we hit our family deductible of $10k in June!
