Anyone familiar with Acclaim insurance?

Can anyone tell me what kind of documentation Acclaim requires as far as past weight loss attempts/weight history? Thank you!

    I was requried to write down all my past weight loss attempts and Dr. supervied I didn't have anything were I had went each month by a Doc to be weighted so I had to do a 6month supervised diet. It passed by really quickly and I got banded in Nov. Feel free to ask me anything if I know the answer i will help.
