what was your insurance requirement 4 documented weigh loss

I wanted to know what did your insurance require you to show as proof of failed diet attempts ect. I get get much out of Kaiser (HMO) so I kinda want to know as I have NO documentation. I have a 37 BMI with Pre diabetes and hypoglycemia.

    my insurance if it covered it was going to require 2 years of weight documentation-like what I had tried-what Id lost etc..... but since I ended up paying for it myself in mexico they require nothing.
