You can obtain this information on the tricare website. I'm a military wife and had my surgery 8/17/2016. You must do the 6mo nutrition classes, at least grp class and have at least 1 comorbidity to accompany a high bmi. However, policy changes all the time. For instance when I started my journey they only covered bypass, right before my surgery I checked again and the covered the sleeve which is what I wanted in the first place.

Tricare is different based on standard or prime and different again by region. I was bmi 35 with two comorbidities. Insurance required psych eval, 6 months supervised weight loss program. Tricare Prime West. Approved within two weeks of completing surgeons requirements.
