BC/BS Why are people paying out of pocket ?

I have been reading a lot of these post about paying out of pocket or co pays. Why is anyone paying out of pocket of the insurance covers the cost. I would go nuts if I had to pay anything additional to what my co pays have been with all these requirements. Does anyone have BC/BS PPO and live in Maryland ? I would love to know if you had to pay anything extra. I'm all types of confused. Please help me understand.

My surgery date is Nov 21 . Went to my final 6mths of medically supervised appointment on 10/13 and got my approval 10/14. Surprised I got approval in 24 hrs, I'm with BCBS-GA. I have also meet my max out of pocket/deductible so I don't have to pay anything!!

My surgery is covered through BCBS of MI so I have been told but all of the copays and deductibles have me stressed. This was not a problem for my new company until I was told that our insurance rolls over now in January and not August as I was misinformed in the beginning. August is our open enrollment date. So I totally understand the concern.
