Surgical Clearance From Primary Doctor?

Has anyone ever had their doctor deny them for surgical clearance?

My Gastro didn't clear me for surgery in September. EGD showed ulcers. I go back next week - and have a new surgery date of 11/9. If all clear, he'll clear me.

My previous doctor wouldn't clear me so I too found a new doctor.

The doctor that I've seeing for like 15 years declined to provide a clearance letter although she had all the pre op checkklist results and pre-operative lab work. Told me my plan doesn't require a PCP, therefore she wasn't mines (when I changed jobs, I picked my insurance based upon the fact that she accepts it). Thankfully, the surgeon recommended a doctor to me.
