I went for over 1 year..because i wanted to try that as my absolute last resort of weight loss before surgery. However it varies by ins.. Most require 1 or 2 visits total i believe.. As far as how close to the surgery date im not sure.

Once per month for 6 months.

I have to have two consecutive visits with some weight loss....no particular amount...just some weight loss. My next visit is on the 22nd of December and if I show weight loss, it will be my last. :D

I had 3 appointments with the bariatric team. I saw the nutritionist at each appt. I also had to take a 2 hour nutrition class.
