Band slippage revision to RNY. A question about insurance!

I was banded back in 2005 and lost approximately 60 lbs. About two years ago I started having issues with my band and gained back a significant amount of the weight I lost.
I had my EGD today in preparation for my surgery and was told my band has slipped. With all the issues I've been having with it, I wasn't the least bit surprised.
I am a bit apprehensive about the RNY but I believe it is my best option.
If you had a slipped band or other issues, was it easier to get your removal/revision approved through your insurance co.? If so, and your insurance company has a waiting period, did they waive it?
