Insurance and deductible payment

Question--I started the process for VSG in October. I am wrapping it all up now. Last appointment on Friday with the nut. Of course as of the 1st the deductible for insurance is reset. Mine is $1000 with max out of pocket of $1000 = $2000. Lol. Does anyone know if you have to pay that amount up front or if it's billed to you? If it's up front I'm screwed! Lol. I have bcbs of New Jersey, Horizon. Thanks!

I had to pay it upfront because it is considered an "elective" surgery.

When I had my surgery I had met all my deductible and out of pocket for the year. I had to pay a $250 co-pay when I pre registered. So far, I have not received any related bills or EOBs in the mail.
