They gave me two packets to fill and one was for the bariatric team to approve or deny me the other was to take to my first appointment if approved. I'm assuming its for them to see if my insurance even covers my surgery if I fell into all the requirement cause it asked for my insurance information and what kind of health issues I have. If anyone else or everyone else had to do this how long does it usually take for them to get back to you? On the paper it said applications are done in order of received and I filled mine out at the seminar but just curious.
