How to add before and after pictures to profile

I found one person who has done this and she can't figure out how she did it. It is very frustrating as I have gone to my profile, I see the outlines where my before and afters should go... i click edit profile, and that disappears.

I know how to put pictures in a gallery but i don't know how to put a before and after as part of my gallery. It can't be that hard but it sure isn't obvious to me!
