Can Someone Please Help Me Get In Contact With Dr Kelly?

I've been looking for information on how to get in contact with this doctor or anyone who works with him. I've sent emails, checked out websites, and have not gotten a response from anyone. I'm hoping someone on here could give me the information I'm looking for?

I would be so extremely greatful.
