Hello,

I have a meeting with my surgeon on Monday.

I was told at the educational class to prepare a Medical Neccessity Letter.

Do anyone have examples or tell me what i should include.

Any help would be greatly appreciated.

Our surgeon's office wrote this for us.. you might want to double check... but if you have to write it here is a good place to start. Physician/Provider How To

My surgeon's insurance manager at his office prepared and faxed my letter after I met all of my requirements . Good luck.

All I brought from my Dr was a Prescription sheet that said: "Cleared for surgery" and then gave my BMI. Done...... :-)
