1) I live in CO

2) fills are $100 after my three months of covered care. My fills are done without fluoro but if I were to get one with, it would be covered by my insurance.

3) My band surgeon does my fills himself.

4) I don't really understand this question. Are you asking if he is banded himself.... no.

1) Florida

2) I was banded in Dalton,GA which would have covered the cost for 1yr...but moved to FL....so I pay $200 w/ Dr Grossbard in Zephyrhills

3) Surgeon

4) Yes he was banded

Banded 3/20/06 -49lbs ;)

1) Area of USA-North Carolina

2) Price with and without fluoro-$250 in-office non-flouro, $300 + about $400 hospital charge for flouro (prices may not be current)

3) Type of Provider, (MD, APN etc)-Bariatric Surgeon, but does NOT do bands (Dr Gonzales in Winston-Salem)

4) Is that provider a "bandster? Nope

I need to know what kind of prices are quoted around the country. Please

include in your post:

1) Area of USA - Dallas, Texas

2) Price with and without fluoro - with $250, they don't do it without I made a $50 co-pay, insurance picked up the balance.

3) Type of Provider, (MD, APN etc) - don't know what APN stands for, my fill was done by a nurse practitioner

4) Is that provider a "bandster? - the doctor isn't but the nurse practitioner is
