Insurances vary greatly. Some don't require a supervised diet and some require 6 months. You will need to find out more specific info about your policy to get an answer.

NWgirl is right. It depends on your insurance. I have BCBS IL and it took a day and a half when the papers were submitted and I only needed a psych eval and BMI of 40+ but my surgeon required everything else I had to do. All in all from start to finish with me it took about a month and half to get done and approved. Good luck! Call your insurance and find out their requirements if you haven't done so already. Keep in mind your doctor may require additional qualifications/testing.
