Letter of medical necessity from Doctor

Does anyone know what should be included in this letter? And what if my doctor says no. I'm 5'7 279 lbs BMI 44

You can look up samples on google. I had to have one of these too. Basically its just your dr explaining to the insurance why he thinks you should have this surgery. Any medical problems, how long he has been treating you, different weight loss attempts. I have a bmi of 44 also, I'm sure your dr. would be on board.If not, do you have another dr you could go to to get it?

I can tell you this from experience.... Aetna insurance sucks! they paid all but 5,000 of placing the ban because it went over Aetnas LIFETIME bariatric cap of 10,000. 6 months later I had to have my band emergency removed and they refuse to pay anything else. therefore I now have a bill of 16,000 that SAetna abolutely refuse to pay so I'm screwed!!!!

my dr had to put he recommends me for surgery because im morbidly obesse and make sure you get a ekg at same time have your dr send that to your insurance or weight loss dr

My surgeon's office had a pre typed letter and my pcp just had to fill in the blanks. You might want to ask your surgeon if he/she has anything similar.
