Medical Neccessity Letter

Hello,

I have a meeting with my surgeon on Monday.

I was told at the educational class to prepare a Medical Neccessity Letter.

Do anyone have examples or tell me what i should include.

Any help would be greatly appreciated.

I've never heard that- any letters of medical necessity should be coming from your doctors office.

Do a google search on the internet for example letters....but yes your primary care physician should be doing the letter, Mine did!! Make sure your doctor lists all co-morbidities in the letter.
