Mine was BMI of 40 with no co morbidities OR

BMI of 35-39 with one co morbidity

Either way you must use an In Network surgeon and Hospital. I was approved in 8 days.

I too have UHC. Mine is through Delta Airlines. They had the same BMI requirements and also required a 6 month supervised diet plan, as well as, a psychological evaluation. I have completed those steps and now just have to find out what other tests/requirements my surgeon has.

Good luck to you!

Mine were the same as jfriddells...Good Luck

Laylas grandma,

Just wondering what other tests your surgeon made you have in addition to the UHC requirements. Do you mind sharing?

My requirements through UHC was

over 21 years old

the BMI requirement as your plan

5 years of documented weight history

and in network surgeon and hospital

Now with that being said........I met all the requirements however i was denied. waiting on a decision for my appeal

they denied me saying i didn't have five years of documented weight history
