Do I need a Referral from my pcp for Medicare?

Do I need a referral from my pcp for medicare? I am overweight and have an existing condition which I was told by a previous Dr that was the requirements. That Dr. is no longer at the local medical center. I have severe low back problems thats why I am disabled and have since gained 60 lbs.
