Letter of medical neccesity examples?

Hello,

Is anyone willing to share what was written is their letter of medical neccessity. The one my doctor wrote was really, really weak and definitely needs to be beefed up. So I was go to re-write it and ask them to sign it.

I want to get this done right the first time!

    I dont have it on here but if you join the YahooBandsters Group they have templates that you can use to show your doc how to properly write one or just print it out and have the doc sign it.

    The Yahoo Bandsters insurance group has them too. Good luck!
