Anyone get the lapband thru Horizon NJ Health?

Can you give me any info like.....

what did they require? like diets, tests, co-morbs?

how long did it take to get approved?

are fills covered?

what wasn't covered?

any complaints?

any other info that would be helpful?

its an HMO thur medi-caid so they may all be the same...?

since the post is older, i was wondering how it went with horizon nj health?

i just started the process thru them, same HMO and everything. they say they cover the lapband, but i was wondering- if you were succesful with them, if it actually did cover anything? ive had nothing but issues with horizon so im kinda assuming this is going to be a long, aggrivating process. haha.
