The Cost of Fills

Just curious. Are fills covered by most insurance plans - if not what is the out of pocket cost for a fill? I am paying for my procedure and up to 10 fills are included in the cost of the package. :confused2:

    My surgery included 3 "free" fills. Since then the Dr. charges me $6.00 a fill to cover the cost of the needle. I don't turn the cost into my insurance since it is so small.
