Does anyone have a copy the Dr. wrote to the insurance company?

HELP !!! My doctor is unsure of all that he needs to cover in the letter to my insurance company in order to get me approved. He says he is even willing to state that I did a 6mth program, even though I didn't!!!! :tt2::smile2:He is so sure this is exaclty what i need in order to get my health back. He isn't sure what is needed in writing to back up his statements, and wanted me to get some insight on this.

Shelly, I am not banded yet but am trying to get everything done for my insurance to pay for it. I think(but am not 100% sure) that your insurance company will require proof of your 6 month diet which would mean a claim once a month for six months from you Dr. You can check with your insurance company and see what they require and exactly how they want it. That is what I am doing. Good luck and I am sure someone else will have more information on the letter to the insurance.

obesityhelp.com has a sample letter and lots of good information about what you need to do in regards to your insurance co.
