Dr. Hilario Juarez Has anyone used him as their surgeon?

My PCP is referring me to Dr. Hilario Juarez as he is on my ins. plan. I have found some helpful info on obesityhelp.com. I was wondering what he requires for his pre lap band patients. Can anyone be of help? He is located in Phoenix Arizona.

    Dr. Juarez.

    I was banded two weeks ago, not by Dr. Juarez, as I am self-pay, but I am very happy with the results and have lost 8 lbs since the surgery.
